How big can the logo be?
Up to about 24% of the code width with error correction Q and 30% with H. A bigger logo would cover too much of the data, so the size slider stops at the safe limit.
Which logo file works best?
An SVG or a PNG with a transparent background. Avoid logos that look like a QR code themselves (three square corners) — they can confuse scanners, and the scan test will show it.
Is there a watermark or a fee?
